Does anyone have any practical experience with Change Management in the SAP environment (ECC 5.0). Defining what constitutes Changes such as table access, Transport/configuration management, Basis work, etc. I'm finding to be more daunting than our other IT functions/disciplines.

Anyway, I don't think that the fundamentals of your change process will be different whether it is in SAP env't or any other environment. If you don't have any change management implemented at all, ITIL Foundations Change Management should provide you with basic elements of what you should have.
For quality control during development, you might look into Application Management module of ITIL, as John (UKVIKING) pointed out in response to Kevin.
Your question is very vague and broad in scope and while I don't have SAP experience per se, the fundamental aspects of setting up a change management process won't be different from any other environment. And in my view, they shouldn't be.
