Hi,

have you had a look at the ITIl books which have something to get started?

Have you also looked at what you are currently doing and what you expect to be doing (guidance again from the ITIl books).

What I have done time after time is always different for each organisation. I can't simply cut and paste a template from one org to another.

Building on what Mark said, the ITILv3 books have a lot of templates and example in the appendices. It would be very beneficial to review those as I have found them to be very helpful.

Get the books and create them youself.

There is no 'template' that fits all.
