Hi all,
I would like to find get your opinion on what all of you think why changes sometimes causes situations. My own opinion:
(1) Poor impact assessment
(2) Ineffective roll-back (back-out plan)
Let me know your thoughts.

it depends
poor process for change management
poor standards as to impact....risk assessment etc
