Completed the Foundation training and certification and currently engaged to move my company forward with ITIL best practices. It is a US based Fortune 300 firm with nearly 1000 IT employees.
My question is...Is it worth the time, effort and expense to gain the ITIL Managers level certification?

Yes.
Beside reviewing in more details the 10 processes defined in ITIL, and their implementation, you will learn how to successfully sell your ITSM roadmap.
If the rest of the organization is not buying it, your ITSM roadmap is doomed to go nowhere.
Oh, and after the certification ... the real hard work still need to be done, and you have no excuses to fail ... as you are now the guru
I completed my Service Manager certification and am in a large organization. Though the certification forces you to build in depth knowledge, I would say that it will not have that great an impact on your implementation ability in the organization.
Also, expect to work really hard to get the certification especially if you have to run things on you day job.
