It is very easy to write a template

Try it

There are no standards as to what has to be in the template and there are no global templates

Pretend you are the person who has to fulfill the request

what information do you need to complete the request

I'm tempted.

I'm tempted to make one up.

But I don't know what you want it to contain.

If you could just list the information you need to have on it...?

And maybe something about the kind of use to which it will be put...?

Perhaps some rules about when to use it...?

... and some rules about how to relate the various pieces of information?

... and the sources of information?

Oh! and something about the format of each piece of information?

Done that?

Okay. The rest is just formatting it how you want it and establishing authority for its use and ...

If you have no idea how to write a template, then how will you have any way of judging whether the one I do for you is any use at all? Will you even understand how to use it?

Please refer to at least two other very recent threads on templates plus some older ones to get a perspective on the problems of not writing your own templates.

Basically if you have not specified it, it will not meet your requirements and if you have specified it then you are 80% (some may say 95%) of the way to having developed it.

OKAY OKAY

1. Title: "IT Service Request Template"

2. Requestor: xxxxxx

3. Date of request: xx/xx/xx

4. Description of request:
[draw box big enough for a few sentences and print in small at the bottom: "see attached documents for more detail"]

5. Date required: xx/xx/xx

6. Authorised signature: xxxxxxxxxxx

7. to x.:

Draw a box marked "IT services use only"] and include fields for date received, assigned enactor, date completed, date accepted by requestor, and any other fields you need to help you control your process.

x+1: document control information according to your document control procedure (or that could go at the top, or part at the top and part at the bottom, but not in the middle - just an opinion).

Now will everyone who wants a template please use this one as a basis. It is easy to modify for most purposes. However it is almost useless unless you do modify it, because I don't know what you really want it for - only you do.

Oops!

I guess I'm not as talented as I thought I was.

I just got frustrated. There was even someone on Linkedin asking the same question today or yesterday and the first response said "me too please".

Still, there are some that won't realize you can change the bits in quotes to suit. And I'm not sure about the font size. Not to mention at least one "deliberate mistake".

I still don't understand how anyone can have difficulty getting at least that far; and once you get that far, its just a case of thinking about what you are doing. Isn't it?

email me and I'll tell you where to send the cheque.

What I did was an incomplete, inadequate generic template with errors in it. If you have no one in your organization capable of doing that or better, then you have problems. One of the primary requirements of any management system is to have people with the skills and experience to perform the tasks required.

It took me seven minutes to knock up that example and I have not worked for over three years. I did not need to look up any books; I did not need to stop and think and probably two minutes was going over it to try to correct typing errors at which I am ver good.

One final time:

I can't - no one can - design your templates without understanding your requirements!!!!!

None of us work at the company that hired you so we dont know what any one in your company may request via a service request


We have supplied you with enough information that should enable you to do what your manager wants

if you still cant seem to work this out, please go to your manager
