It appears to me that you have no knowledge of ITIL, IT Service Management nor of Change Management
The questions you are asking are not advice - which is always free - but an attempt to get consulting from the forum
My question to you is as follows
1 - Do you have any experience or knowledge about Change Management
2 - Why are you asking this forum on how you should implement Change Management

You may have experience in CHange Management in the same way I have experience in medicine by going to the doctor and discussing my illness or condition.
You obviously have no experience or Knowledge of implementing or managing Change Management. ALl of your questions have to deal with the basic of Change Management. In fact, all of your questions can be answered / resolved by reading the Change Management section in the ITIL Books. ..NOTE: Please dont compound the issue by asking which book.

You obviously have been thrust into a role or an area where you have
no experience of the correct type.
You need to go to your management and get the following
1 - ITIL Foundation course.
2 - Practitioner course / Intermediate course in ITIL for Change Management
3 - CoBIT Foundation course
3 - ISO 20000 Foundation course (optional)
And if you cant do that, hire some who has the right experience
While every one who visits this site and can ask questions about ITIL and ITIL related areas; the usres are expected to be some what knowledgeable about the subject matter and desire advice (free) or a clarifiying or an opinion
