Currently we are looking to enhance our current RFC form. I was hoping to find some templates or get pointed in a good direction for templates. Our current information:

Description, Implementor, Requestor, Approver, Change Date, Type(standard, non or emergency) Impact (High, Medium, Low), Comments, Systems effected.

Are planning to add: Completed, Successful, Unsuccessful, others?

I have a paper template for an RFC and also one for the Release document (also includes the Review doc) that I am more than happy to share - Let me have your E-Mail address and I will forward them to you.

sorry folks - I've realised that my last response was less than helpful to the rest of the forum - so here goes

Our RFC contains

Change Type : Platform : Machine Name : Priority : Category : Summary of Change : Business Impact : Information Security Impact : Risks : Mitigation of said Risks : Testing Methodology : Planned Date & Time : Regression Methodology : Sign-Off & notes on why people are signing

Our Release Notice contains

Implementer : Start Date & Time : Finish date & Time : Implementation Instructions : Regression Procedure : Testing Results : A Form for noting Serial nos etc. etc etc for Hardware Installs also includes details of back-up frequency, escalation paths, Maintenance Contracts, power requirements : Sign-Off & notes on why people are signing : Review Checklist and Sign-off.

In simple terms we have split the Business case away from the technical processes required.

These are in the form of Word document templates.

I hope this sheds some light - I am sure there are things we should have included, but haven't and some people will question what we have. I am interested in any feedback.
