Hi All,
I am trying to implement Change Management at my company. I am not sure about how many stage i need to have in Change Management and in what order. The stages i thought that i need are
[list=]Requested[/list]
[list=]Approved[/list]
[list=]Implemented[/list]
[list=]Scheduled[/list]
If I have approval at multiple stages, i.e, during scheduling, implementing etc, then how do i set the stage, or should i be having only one approval for a change.
What other stages i might be needing. Can someone give me some detials on this.

Don't forget to add Review at the end!
Changes do not go smoothly all the time, and you need to be able to learn from errors / accidents.
This is all part of Continous Improvement.
I only have three areas of sign-off
At the RFC stage (Business case)
at the Implementation stage,
and at the Review stage
The only common people in these are the Implementer and the Change Team who sign at all stages.
As far as the order goes - I would say the following:-
Requested
Scheduled (added to Forward Schedule of Change)
Approved
Implemented
Reviewed

Thanks Ed,
Does 'Implemented' mean that the change work is over and that it has to be tested or does it mean that it is tested already. If it the former, does it mean that i need to have two more stages called testing and completed.

Implemented means 'put live in production'
you could add them, but I missed them out as they are part of Building / Testing the change not part of the Change Management .
I can send you a full, but simplified flowchart that shows it all if you wish!
There are in fact something like 20 steps in all, if you want to go to individual steps like testing (without any iterative steps counted more than once)
I am sure that I could probably get it up to 60 or 70 with a typical change.

post implementaion review

I would love to get the flow chart as well and compare as to what we have implemented for our change management process.
We currently use this:
Requested - initial submission to change management
Accepted - RFC has been correctly filled out and understood
Reviewing - RFC is currently being reviewed
Waiting - RFC is on hold or waiting due to more info being requested
Approved/Denied - RFC has been approved or denied
Scheduled - RFC has been authorized to be implemented at stated date
Implemented - RFC has been implemented
Completed - RFC has gone through PIR
One question is what if the you needed to roll back the change, didn't go as planned, what status do people then use or process to indicate that RFC failed?

We would close those changes as unsuccessful or backed out. The PIR would look at what was learned. |

Once the Planned Activity is completed. It needs to be closed with any one of the following Options (Depending on: Successfull/Failed Change)
Implemented as per request
Implemented but exceeded planned duration
Implemented but caused later incident(s)
Backed-out as change did not work
Backed-out as exceeded planned duration
