I have recently taken on the role of Change Manager within my organisation and I have so far gone through the hoops of the ITIL Foundation course. I am now trying to create a decent template for an RFC that would form the basis of my challenge in creating a good Change Management process here.
Does anyone have any of these that I can see please?

It is by no means the final answer, but we think it covers most of the things you will need.

All
I will send a copy to the admin account, and hopefully the thread will stop for people just wanting my version of an RFC.
I am happy to discuss the merit/demerits of my document, so if we want to continue the thread on that basis keep posting.
I would like to point out that these templates are for out limping paper system for Change Managent, and the Release side is quite rudimentary (before you all tear me to bits)
