Hi,
I am a PM with about 17 years of IT experience living in USA. I would like to certify myself in ITIL. Following are my queries:
1) How do i go about.
2) Whom should i reach out to in USA
3) Which certification i should take from the following:
The ITIL Foundation Certificate
The ITIL Practitioners Certificate
The ITIL Managers Certificate
4) What is the cost and where do i take this test in USA.
5) What are the prerequisities to take this certification.

You need to pass the managers exam first. It can be done self-study, but a 3 day course is better.
Pre-requisites for Foundation are minimal - that you work in IT, basically. You cannot go on to the other qualifications without having the |Foundation,. For them the courses are mandatory, as is more IT expeience (5 years management experience for managers).
You sit the Foundation exam as part of a course or at a Pro-metric or pearson Vue test centre. Practitioner exam is part of the course, Managers exam is a public exam every quarter.
Remembe that it is all changing with V3.

Step 1: Clear Manager Exam - take test in a prometric centre - avle every quarter
Step 2 : Clear Foundation Exam -- take test in a prometric centre
Step 3 - Clear Practitioner Exam - take test in a prometric centre - avle every quarter

No - you are incorrect.
Step 1: Clear Foundation Exam -- take test in a prometric centre
Step 2 (optional0 : Clear Practitioner Exam - take test at the end of a course at course provider's location
Step 3 - Clear Manager Exam - take test in a venue arranged by your course provider (if you are outside the UK) - avle every quarter _________________ Liz Gallacher,

I believe your confusion may be from a typographical error that Liz made:

That should read:

Although you can self study for the Foundation and take the course at any Prometric facility, I would recommend that you find an EXIN certified training organization and take the 2-3 day course.
The path forward after passing the Foundation exam is varied. You can then sign up for any of the EXIN Practitioner courses:
IT Service Management Practitioner: Release & Control
IT Service Management Practitioner: Support & Restore
IT Service Management Practitioner: Agree & Define
IT Service Management Practitioner: Plan & Improve
IT Service Management Practitioner: Security Management
IT Service Management Practitioner: Service Level Management
Or go straight to the Service Manager course. The Service Manager course is not an equivalent to taking all the Practitioner's courses. The Service Manager course focuses on the integration or "touch points" between all the aspects if ITIL where the Practitioner courses are the nitty-gritty, focusing on two or three aspects of ITIL.
I previously was a trainer for PinkElephant and have taught the Foundation and Practitioner courses of Release & Control and Support & Restore. I believe they now offer a course for Agree & Define. They also offer the Service Manager course. Being familiar with the staff and materials they use, I can heartily recommend them. But I can't give a unbiased opinion having never received training from any other organization.
