Change Management Survey

I would like to get feedback from within our organization about what they think about the change management process. What is working well, what isn't, how is the tool that they are using to submit RFCs, etc...

Is there a set of standard questions that someone else has used or have done this before?

The OGC had a questionnaire-based self-assessment tool that included each discipline of ITIL V2 service delivery and service support. Various consulting firms have their own ITIL maturity assessments but these are usually proprietary.

Search the web for "itil self-assessment".

I think that one is geared to determine how ITIL you are

I just di one with my customer base but limited to the CAB etc

Ask them

Are they involved with CM
Do they know the CM policy
what role do they take
ado they go to the cab
do they receive the FSC and other deliverables.
