I am trying to re-educate my IT teams and want them to start using a
Rollback Plan Template for all of their change rollback plans.
Does anyone have one that would save me some time?

Not a template, but for starters here are some things you might want to think about\include:
Q. What circumstances will necessitate back-out and at what point will a decision to back-out be made?
Q. Who will make the decision to back-out?
Q. How will the back-out be performed?
Q. What contingency measures have been considered?
(e.g. backups to be taken, available workarounds.)
Q. What is the expected time required to perform a
