Currently in our organization we have a form that the user fills out which calculates out the score of their change request which gives a Category of Minor, Signifficant or Major. It was our first run of a more detailed assessment and now we are looking to update this form again to fine tune assessments. Would anyone have examples of similar forms with weighted scoring? Our environment is a very distributed infrastructure in the way that releases happen, we have a large number of retail and corporate locations and a bit difficult to come up with a form that covers both worlds. So just be interested in seeing some assessments other organizations are using and how we may possibly can piece in additional assessment questions that we are not thinking of.

We collect inputs from change request form for assesment and impact analysis as below
1) classify the changes, for example changes may fall in any of the caterogories... like bugfix,upgrade,enhancement,new service ....etc, 2) change type may fall in 'Simple', 'Minor','Major', 'Medium' 3) Impact type may fall in 'no intteruption', 'simple interuption', 'major interruption'. 4) Downtime required 'Yes' , 'NO'...... list goes on like this.

Hope this gives an idea.
