Currently the examination boards are EXIN and ISEB (APMG were just awarded the new accreditation contract for ITIL so this may or may not change next year) .
The certification path is one of:
ITIL Foundation
ITIL Foundation > Practitionar (in a specific process or group of processes)
ITIL Foundation > Manager
ITIL Foundation > Practitionar > Manager
(Note: In the US they call the Manager certification a Masters)
From the above you can see that the Foundation is a pre-requisite to all else.

Also: the foundation course will mainly give you definitions and a general overview of ITIL (some people call it the 'theoretical part', but it is a mistake to call anything about ITIL 'theory'.
the practitioner courses will get you up to speed with one process, where you will receive a lot of hands on training and role playing on a particular process.
The service management course will deal with implementing ITIL.
