The way I always understood the different goals, the practitioner exams are meant for professionals in a certain area who want to extend their knowledge of the area in which their professional interest is situated, and in that view the Service Manager certification programme would be for people who are dealing with most processes in a general and overviewing role.
Personally, I have only recently acquired a position as service management consultant, but I have approximately between 4 and 8 years experience with service management and ITIL, depending on the process.
I have passed my Foundations exam, but I am a bit puzzled as to what my next step should be.
If you have a role as a service management consultant and are expected to have an overall understanding of the processes and perhaps implementing them, then I would say you should go for the Manager's Certificate which consists of exams in Service Support and Service Delivery. You can view syllabuses for these on the ISEB and EXIN websites.
