I have just relayed to the various IT departments around my company that I now want them to start completing PIRs as part of the change process. I have been asked whether the PIR should apply to the Standard Change process as well. Although I am new to Change Management I would respond to this in the affirmative as even Standard Changes may at times, have some adverse affects that were not considered at the time of agreeing the Standard Change model.

I would not expect to conduct reviews on Standard Changes - If the process is badly understood, then the Standard Change should be withdrawn/refused. The whole point of Standard Changes is that the process is well understood, all the steps are known, and all the risks are understood.
I ask my techies to write a procedure for their Standard Changes. If they can't, then I do not grant it.
