Hi,

I was wondering if anyone has a template they could share that helps document a post mortem for a major incident. I have developed a few but they seem to be a bit complex for our business partners. I am looking to see if anyone has put together a nice simple template. Any help would be greatly appreciated.

Regards,
Ryan

Ps. I would be happy to share any templates I have with anyone interested.
