Creating, updating, terminating an so on are all some changes of the IT environment. So all these tasks must go through the change-process. There shoud also be a service-ower (leader of user-support) who confirmed these changes and if necessarily you have to update the CMDB.

Set up a user from my point of view should be managed by Change Management but shoud be treated as an standard change. Of course this change should be saved in CMDB _________________ --

Another perspective is to think of this as part of SLM. It would fall under the area of Service Provisioning (providing service to a customer). SLM would utilize Change Management to make the actual updates, but the driving force would be the SLA and the delivery of service.
