I am looking for preparing KEDB Process Document. The document should have details about SIPOC or Process Diagram, Phase Diagram and further the phase diagram should be broken into Activities along with RACI matrix.

I have prepared a draft version but was getting stuck at the stage where we need to break down the activities...was just looking at approach and some of the important points that needs to be covered

I mis-read your original post thinking you wanted some one to create the diagram and present it to you
Instead, what you are saying is that you are stuck
My advice is as follows
CoBIT Mapping to ITIL at isaca dot com
This has a good list of RACI charts and activities
you have to join the site but I cant recall if you have to join ISACA
Ask your self what the purpose of the KEDB is and how the data will get there, who will use it etc
write these down - not necessarily in order
go back and organize them and then expand / contract on them
these will get you an idea of the activites
