What policy document do you have in mind?
Speaking generically, the key elements are:
1. the scope of the policy's applicability
2. the ownership or authority of the policy
3. the policy statement itself
4. the date of establishment as policy
... and, unless it is covered in a higher level policy, the arrangements for review of the policy.

firstly, apologies, I forgot "title" which is obviously a key element.
Secondly, policy defines the culture of your organization, or at least it is derived from the culture. I cannot see how anyone's sample policy could help you.
Policy is one area where I am very much in favour of top-down. I cannot see how to define a policy for SLM or configuration until you have top-level policies for your organization. That at the very least requires a quality policy for Service Management before you consider SLM etc.
