I'm pulling some standard questions together for terms of reference for a Release Post Implementation Review and could use some suggestions. So far I've got:
Was the release deployed in line with the plan?
Did the release follow expected deployment timescales?
Did the release generate any major incidents?
Were all expected resources available for the release deployment?
Did the release generate any subsequent emergency changes/normal changes?
What went particularly well?
What coud we have done better?
Any others that anyone can suggest? There is less of a focus on cost of releases so I've shied away from the 'on budget type questions'. Also the focus here is very much on the deployment of the release rather than the quality of its contents (something we're working to change).

- were the planned resources appropriate/adequate (scale and skill and availability)?
- something about communications.
