Your question is very generic .

Please be more specific so that we can help you better.

Always ensure that you are capturing the below when you are involving yourself in PM process

1.Detect the Problem and Log the Problem

2.Categorize the Problem and Prioritize

3. Investigate and Diagnose

4.Have a Work Around logs and known errors captured at a location

5.Resolve and Close

6.Major Problem Review and avoid reoccurence
