Can a change be backed out on it's original ticket if it's past its implementation end time? Is a new change ticket then required? Or is a change record required if the change caused an IM? Can the change be backed out on the IM ticket?

IT Depends

If the solution which during the post implementation steps during the change window is found to be not fit, then the back out plan should be invoked.

This of course depends on if there is a back out plan
this of course depends on if the window for doing so is included in the change window
this of course depends on if the above was discussed at the change board
