Is there a formal ITIL "Data Model"/"Entity Model" out there?
This would be an Entity/Relationship model or else a UML Class Model, describing the concepts of ITIL in boxes-and-arrows form.
E.g., concepts like "Service Level Agreement", "Key Performance Indicator", "Requirement" would each be an entity; their attributes (fields) and the links (relationships, associations) between them would be shown.

I haven't seen anything like that so far, but all these elements are normally clearly described in the ITIL books (not so clearly regarding ICTIM though...) and should be adapted depending on needs and/or organization.
The only thing I can think of which is somehow similar would be IPW Quint model:
