Is someone can help us to have an IT Capacity Plan template ?
It's a document used when implementing a capacity management process and it lists the current level of resource utilization, service performance and future requirements.

Why cant you write your own ?
A template that is written by someone else will not meet your requirements and you will have to edit it quite extensively
In addition, have you tried google searches as there should be a lot of examples on the net

Check out Appendix J of the ITIL v3 Service Design book - you'll find guidance regarding typical contents of a capacity plan.
