Hi, does anyone have any idea where I could get an ITIL approved Service Take On document?

I do not know of the document of which you speak (does it exist?), but i can assure you that ITIL does not provide any "approved" operational documents.

I think you need to determine your requirements and generate this yourself.

I'm having difficulty deciding whether "ITIL approved" is a contradiction in terms, or just a non sequitur, or merely a logical inconsistency.
